The YWCA recognized Vice President of Care Continuum and Wellness Denise Schrader with its Women of Achievement Professional Award. The award is presented to women in the fields of medicine, law, education, science, engineering and others which require professional accreditation in order to perform work in that field.
Schrader’s nomination describes her as a leader in the home care/hospice industry, nationally and locally, with a reputation for producing results. She is called upon by national and state associations to advocate for legislative and policy changes improving access and quality of care for critically ill and elderly patients.
Schrader has implemented new technologically advanced program services. She led the Health Care Reform Steering Committee for Rockford Health System, successfully creating a clinically integrated network, Medicare ACO and bundled payments to better the patient experience, reduce costs and improve clinical outcomes. She also developed a robust post-acute care team focusing on care outside the hospital in nursing homes and physician home care services to improve care across the continuum.
