Active Aging presentations will be offered the first Thursday of every month. Whether at home or in assisted living facilities, the ultimate goal for both is to provide excellent patient care and arrange appropriate services for the patients and their families.  Join Mohammed Ahmed Khan, M.D., the director of Palliative Care and Advanced Illness Management Services at Rockford Memorial Hospital, as he discusses both options to help you have the best quality of life.
